In order to be considered for the VA Hines Dietetic Internship Program you need to submit a completed application packet. Only U.S. citizens are eligible to apply. The application must include the following:

  1. Letter of Application (typed). Should include:
    • Reasons you selected this internship
    • The area(s) of dietetics you are most interested in at this time and why
    • Examples of your organizational skills, work ethic, interpersonal communication skills with patients/customers, peers, and supervisors, and enthusiasm/dedication to the field
    • Future goals
  2. Dietetic Internship Application Form (Use the standardized application form of the ADA which can be downloaded from this website.)
  3. Three References
    • Use ADA recommendation Form (enclosed)
    • Two references from professors in Food and Nutrition or related area
    • One reference from a supervisor of a work experience

    NOTE: Reference forms must be sent in sealed envelopes with the signature of the reference person across the flap. Make sure the applicant's name is on each reference form.

  4. DPD Verification Statement or Declaration of Intent
    • Verification Statement signed by DPD representative confirming completion of academic requirements
    • OR

    • Declaration of Intent signed by DPD representative with list of courses to be completed

    NOTE: Applicants must be eligible for completion of the Dietetic Program in Dietetics--with degree awarded prior to the start of the internship. An original signature of the DPD program advisor is needed.

    If the degree was granted five years ago from date of application, additional courses in dietetics or a related area are required to update the degree. Courses taken to satisfy the requirements must be completed at a U.S. regionally accredited college or university. An official transcript verifying completion of the courses with a grade of "C" or better must accompany the registration eligibility application. Only courses completed within the last five (5) years will be acceptable. Courses should include, but are not limited to Foodservice System Management, Advanced Nutrition or Medical Nutrition Therapy, Human Physiology, and Community Nutrition.

  5. Official transcripts from each university or college attended
    • Only original transcripts accepted; or college attended
    • Mailed directly from registrar's office OR sealed in envelope with registrar's signature across the flap
    • All Fall semester or quarter grades must be included for application to be considered complete.

A physical is not required at the time of application. However, a dietetic internship appointment is dependent upon completion of a physical verifying acceptable health status prior to commencement of the program.

Receipt of application will be acknowledged if a self-addressed postal card is included.

Incomplete applications will not be considered, so please check all your materials carefully. We are unable to return application materials.

Note: NO application fee is required.

All applicants to accredited Dietetic Internships (DI) programs must participate in computer matching. Applicants should request instructions and a mark/sense card to prioritize their DI preferences. Applicants should request this material from your Didactic Program Director (DPD) or from D & D Digital Systems. This request should be made to allow turn around time for submitting by a February 15, 2008 postmark deadline. There is no charge for this material. However, there is a $50.00 charge for computer matching that is due with the applicant's prioritized ranking. Any questions about the computer matching process, applicant responsibilities, and deadlines can be obtained from the D & D Digital Systems web site www.dnddigital.com.
